The Employee Experience Team will help you or your loved ones with next steps and provide support after a death, including filing for any survivor benefits. There are also additional resources through the EAP to help you at your time of loss.

Life Insurance (Personal Accident) | Declare your nominee by logging in to the UHCP portal. If a nominee dies, submit a request via the Support Centre or the UHCP team to update the nominee information within 30 days of the death.
